•	Provide patient medical services, which are limited to services within the training and experience of the practitioner, services in accordance with the Protocols of Patient Care, as amended, and in accordance with other policies and procedures as approved by the Delta Memorial Medical director, Administration and applicable state laws.  The Delta Memorial Medical Director will be the collaborating physician in accordance with Arkansas Medical Board and Arkansas Nursing Board.   

•	Obtain complete, detailed, and accurate medical histories on patients.  Review patient records to develop comprehensive medical status reports.   

•	Performs health assessments and physical examinations, and records all pertinent patient data.   

•	Collaborates with physicians and other health professionals.  

•	Interpret and integrate patient data, for the purposes of establishing presumptive diagnoses, and the general workup of the patient by ordering appropriate laboratory studies, radiological and other special examinations or diagnostic procedure required for the evaluation of illness.  

•	Synthesize data to determine a patient assessment and a therapeutic plan in accordance with established protocols.  

•	Perform therapeutic procedures in accordance with clinic treatment protocols and as authorized by the medical director.  Within the scope of practice authorized, adapt and apply the principles, methods, and techniques of medicine to provide quality patient care.  

•	Administer injections, immunizations, and oral medication.  Prescribe, administer, and dispense drugs in accordance with federal and state laws and standards of practice.  

•	Provide instructions, guidance, and counseling regarding medical care matters to patients and their families.  

•	Arrange for referral or refer patients as needed services that cannot be provided to patients in the clinic.  

•	Interview and advice patients regarding health and illness prevention.   Recommend community resources to meet patient and family needs as appropriate.  

•	Assists physicians working in the clinic as necessary.

•	Deliver medical care services to patients at other locations, as authorized by the medical director and the clinic director.  

•	Triage patient phone calls and provide consultation, as requested.  

•	Perform independent evaluations and treatment procedures in emergencies.  

•	Monitor patients and provide continuity of care between visits.  



•	Accurately and thoroughly document medical visits and procedures as they are being performed via the electronic documentation system, including but not limited to:  
o	Patient medical history and physical exam.
o	Patient subjective and objective data related to visit.
o	Assessment and plan of care for that visit.  
o	Procedures and treatments performed 
o	Patient education and explanation of risks and benefits
o	Diagnoses, prescriptions and instructions for patient or family members for self-care and follow-up

•	Participates in performance improvement activities as requested by clinic director.       

•	Reports to clinic director of any customer complaints, system problems or any situations that affect the day-to-day operation of the clinic in a timely manner.   

•	Ensures that all clinical data, lab or other test results have been reviewed and interpretation of the results are recorded accurately in the medical record.

•	Spot mistakes or inconsistencies in medical documentation and check to correct the information in order to reduce errors.    

•	Assist in maintaining systems to track patient follow up, preventative care and compliance.

•	Documents all communication and coordination of patient care in electronic documentation system.  

•	Be the system navigator and point of contact for patients and families, with patient and families having direct access for asking questions and raising concerns.  May assume advocate role on the patient’s behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion.

1.	EDUCATION	Graduate of an accredited school of nursing and from an accredited nurse practitioner/physician assistant training program.  
2.	LICENSURE	Current RN and current Nurse Practitioner or Physician Assistant Certification.  Current Arkansas Nurse Practitioner or Physician Assistant furnishing number (DEA) and Valid National Provider Identifier (NPI).
3.	EXPERIENCE	Two years of experience preferred.   
4.	SKILLS	Knowledge of professional medical practice and care to give and evaluate patient care.  Knowledge of organizational policies, regulations, and procedures to administer patient care.  Skills in taking medical history assessing medical condition
