Responsible for the management of the Delta Memorial Hospital (DMH) Pharmacy ensuring adequate provision for pharmaceutical services regarding the procurement, storage, distribution and control of medications in accordance with all applicable state and federal regulations; the CMS Conditions of Participation for Critical Access Hospital Pharmacy; and the laws and regulations of the Arkansas State Board of Pharmacy. This is a senior clinical services management position with wide latitude for the exercise of independent judgment subject to the DMH governing officials and regulations.


Responsible for the direction of the DMH pharmacy to ensure efficient and effective delivery of quality pharmacy acy services. Maintains appropriate records, forms and reports as may be required by CMS Conditions of Participation for for Critical Access Hospitals and the Arkansas State Board of Pharmacy. This task is performed directly &
through supervision of subordinate pharmacy technicians/nursing staff employed by DMH.

Provides ongoing and systematic monitoring of hospital pharmacy inventory to ensure pharmaceuticals are
properly stored in accordance with acceptable standards of pharmacy practice, Arkansas State Board of Pharmacy and and CMS Conditions of Participation for Critical Access Hospital Pharmacy.

Oversees the DMH Pharmacy and Therapeutics Committee. In collaboration with the DMH Medical
Staff, maintains a complete and accurate formulary for hospital pharmacy. Conducts periodic education &
training for both medical & nursing staff concerning pharmacy services as requested by the Chief of Medical
Staff or the Chief Nursing Officer and/or as may be recommended by the DMH Pharmacy and Therapeutics

Reviews and approves all purchase orders for pharmacy products used by DMH through approved vendors.
Keeps hospital administrator informed of the cost of hospital pharmaceuticals in comparison to AWP. This
task is performed through supervision of subordinate pharmacy technicians/nursing staff employed by DMH.

Compounds and dispenses prescribed drugs in a healthcare facility environment.

Responsible for transferring prescriptions, handling incoming physician calls, logging data into the computer and Drug Utilization Review.
Develops the annual budget and monitors financial performance of the department on a monthly basis
against established budget.
Provides ongoing and systematic consultation to the DMH administrator regarding the budget, capital
equipment needs and staff needs for DMH Pharmacy services to ensure that services are provided in an
efficient and effective manner.
Maintains and updates DMH Pharmacy Policy and Procedure manuals an annual basis or as needed.
Works collaboratively with other department managers to coordinate pharmacy services
Acts as a liaison between hospital administration and co-workers; interprets policies, and supports change and decisions of administration. Directs questions and concerns through the appropriate channel of
Actively participates in the Performance Improvement program at DMH.

Attends monthly department head meetings and other hospital meetings as appropriate.

Participates in hospital sponsored community activities

Responsible for the selection, training, supervision and evaluation of all persons employed by the DMH
Pharmacy in accordance with the DMH policies and procedures. Such responsibilities include, on at least an
annual basis, a written performance evaluation of all subordinate pharmacy staff.
Maintains personnel records of appropriate licensure, attendance and counseling, etc for staff. Maintains
sufficient numbers of qualified staff to assess and provide pharmaceutical services to the patients. Interprets
and maintains staff compliance with hospital and system policies. Maintains sufficient and appropriate
educational programs to keep staff up to date with current practice. Encourages and participates in the
formal and informal development of staff at all levels.
Develops schedules for staff. Develops and monitors staffing patterns appropriate to the scope of care
and the technology employed in the Pharmacy.
Assesses the patient following established pharmacy standards of practice and using clinical knowledge to
determine an optimum pharmaceutical care plan.
Provides pharmaceutical education to patients and/or family member consistent with current clinical knowledge
and standards of practice.
Maintains strict confidentiality in accordance with the Hippa Confidentiality Agreement.

The pharmacist shall be on site at least five (5) days per week to perform and review pharmacy dispensing, drug utilization and drug distribution activities. A pharmacist shall be available to
provide emergency services to the staff when the pharmacy is closed 24/7.
