Patient Outreach Specialist
• Will identify high-risk patients with at least two chronic conditions, expected to last 12 months or longer; and discuss the CCM services available with those patients.
• Will play a key role in connecting patients with the care and resources that are needed.
• Document all outreach attempts and patient interactions in the electronic health record.
• Will support patients in navigating healthcare services to improve outcomes.
• Will obtain advance consent for CCM services to ensure that the patient is engaged and aware of applicable cost sharing. Consent may be verbal or written and documented in the medical record.
• Will scan signed CCM agreements in the patients EMR.
• Mail Welcome letters and Care Plans to CCM patients.
• Will keep a excel tracking spreadsheet up-to-date with the current CCM patients and report this monthly to the CCM Coordinator.
• Will assist the CCM staff to provide chronic care management services, at least 20-60 minutes of clinical staff directed by a physician, per calendar month, with the following elements: multiple (two or more) chronic conditions, chronic conditions place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline.
• Will add CCM patients monthly to the CCM track board.
• Complete the registration or check-in-process of each patient, correct, up-to-date information, include insurance eligibility verification.
• Enter charges at the end of the month for cumulative time and codes.
• Responsible for providing administrative support to the CCM and the Behavioral Health staff.
• Clerical and administrative tasks related to patient care such as scheduling appointments, reminder calls, outreach letters, coordinating integrated services, updating patient contact information, etc.
• Assist with care coordination by connecting patients with needed resources, obtaining ROI’s as needed, etc.
• Assist with coordination of Integrated Care Services by monitoring and working with various disciplines.
• Adheres to all approved company processes and procedure including documentation protocols and best practices for daily work logs, interactions with patients and providers, and internal communications.
• Will demonstrate effective communication, which is fundamental to the existence of the clinic therefore it is important to respond to emails, electronic communication messages, voicemails and any form of communication that you receive in a timely manner.
• Documents all communication and coordination of patient contact in electronic documentation system. Assures documentation includes tracking and time stamping to support billing for CCM.
• Establishes and maintains positive relationships with all assigned patients and corresponding providers.
• Contributes as a positive team player within the department by supporting all members of the team in a productive and constructive manner. Acts as a resource to others when needed.
• Adheres to all approved company processes and procedure including documentation protocols and best practices for daily work logs, interactions with patients and providers, and internal communications.
• Provides orientation to new team members as directed.

• Performs other duties as assigned.

4. SKILLS Experience working with an EMR system and ability to record activities into an Excel tracking
Spreadsheet. Excellent attention to detail; able to maintain complete and accurate documentation of all patients related to activities. Ability to work independently, with minimal supervision. Highly motivated, results-oriented with strong skills in customer service, organizing and multi-tasking. Flexible to handle fluctuating workloads and tasks.
5. INTERPERSONAL SKILLS Excellent written and verbal communication
skills, particularly on the phone. Ability to build relationships and quickly develop trust with patients and healthcare professionals.
6. PHYSICAL EFFORT Requires finger dexterity, eye/hand coordination and full range of body motion including the ability to stoop, kneel, crouch, reach, push, pull and grasp items and stand and walk for extensive periods of time; the ability to lift and move equipment/supplies (0-25 pounds) with proper technique and accessories. Requires corrected vision and hearing in normal range.
7. HOURS OF WORK Monday –Friday with extended hours as needed.
8. HAZARDS Exposure to chemicals, machinery, and bloodborne pathogens that requires the use of personal protective equipment, i.e., masks, gowns & gloves.
9. COMPETENCE FOR SPECIFIC
PATIENT POPULATION: Infant, Pediatric, Adolescence Adult and Geriatric
10. INFECTION CONTROL: Ability to comply with department specific controls implemented to reduce the risk of acquiring and transmitting infections within the healthcare setting. The controls may include, but are not limited to, immunity to certain vaccine preventable diseases, screening for tuberculosis infection or disease, the ability to utilize indicated personal protective equipment and attendance at appropriate training.
